The first of two 4K In-person Registration days is happening Monday, April 15 from 8 a.m. to 6 p.m. at the OASD Administration Building (215 S Eagle Street). Registration for the OASD’s 4K program is a two-step process.
  1. Families must first complete New Student Enrollment online. If you do not have access to a computer or need assistance with New Student Enrollment, we will have computers and staff available to assist you at In-person Registration.
  2. The second step is to attend In-person 4K Registration on either April 15 or May 2 at the District Administration Building anytime between 8 a.m. and 6 p.m. (no appointment necessary).
Every family must attend In-person Registration to reserve a seat in 4K. At this time, program staff will collect any outstanding paperwork and discuss 4K placement. (NOTE: Students with an IEP or families planning to enroll in Head Start should refer to the specific individualized directions previously provided.)

The following will need to be uploaded during New Student Enrollment OR verified at Registration:
  • Proof of Birth Date: Legal documentation that contains student’s full name, birthdate, gender, place of birth, and parent/ guardian information (e.g. original birth certificate, hospital record, child ID card, Native American Tribal document).  A child must be four years old on or before 09/01/2019. State law requires that the birth date be verified at the time of enrollment.

If parent or guardian is not listed on the documents, however he/she is married to the owner/ leaseholder/ renter, a copy of the marriage license must be provided.
  • Confidential Student Immunization Record: Be sure to enter the complete date (month-day-year, e.g. 03/01/98) or bring a copy of your child’s immunization records. Note: Parents can access the Wisconsin Immunization Registry to obtain a copy of their child’s immunization records. (www.dhs.wisconsin.gov/immunization/wir.htm) Wisconsin state law requires this information.
  • Health Examination Record: Please have your doctor complete and sign this form based on your child’s four-year-old check-up. They are welcome to fax it to the Office of Early Learning. If your child has not had their four-year-old check-up yet, please ask your doctor to fax it after their appointment. We will need to obtain a copy no later than 09/30/2019.
